Job Description

Position Overview:

The Patient Safety Officer role will involve working under the direction of the Patient and Quality Lead to assist with incident management and quality improvement work as well as providing practical day-to-day specialist advice and support relating to patient safety. This position serves as the primary advocate for patient safety and continuous improvement throughout the organisation.

Key Accountabilities:

  • Work directly with clinical teams and specialties to ensure the implementation of incident reporting systems, providing facilitation of root cause analyses (RCA) and investigative support where appropriate.
  • Design, implement and oversee comprehensive patient safety program.
  • Develop and maintain patient safety metrics and reporting systems.
  • Coordinate the Hospital Morbidity and Mortality Committee.
  • Provide education and training on patient safety principles and practices.
  • Maintain the Integrated Risk Register and collaborate with clinical departments to identify and address safety risks.
  • Track and monitor the implementation and completion of action plans arising from incident report investigations.
  • Conduct and facilitate Failure Mode and Effect Analysis.
  • Ensure compliance with regulatory requirements related to patient safety
  • Review incident reports and identify trends requiring intervention.
  • Implement evidence-based practices and protocols related to patient safety.
  • Ensure that communication and systems and processes are in place to support the integrated governance processes with colleagues in investigations, complaints and litigation.
  • Work with the Patient Experience team, Quality and Legal Service team to identify areas of patient safety arising from complaints/concerns investigations which need to be addressed.
  • Work with the specialty teams to develop a systematic approach to the communication of lessons learned from incidents and complaints and to support a philosophy of open disclosure.
  • Serve as a liaison with external regulatory agencies on safety matters.
  • Any other responsibilities directed by the Director of Clinical Governance.

Education & Qualification, Experience, Training & Knowledge:

Education & Qualification

  • Bachelor’s degree in Nursing is required.
  • Masters degree in healthcare is preferred.
  • CPHQ/CPPS is desirable.

Experience

  • At least 3 years of clinical nursing experience.
  • Experience in management of incidents, including conducting case reviews, RCA, FMEA and risk assessments.

Training & Knowledge

  • Extensive knowledge of the principles of patient safety and risk management principles and application.
