Coordination and Communication

  • Serve as the primary liaison between the physician, pharmacist, and bedside/hemodialysis nurses regarding all medication-related matters.
  • Receive and clarify medication orders from the nephrologist, ensuring accuracy before dissemination to the bedside nurse.
  • Coordinate medication changes, dose adjustments, and discontinuations across all relevant teams to prevent errors or duplication.
  • Ensure all staff involved in home dialysis are promptly informed of medication updates or safety alerts.
  • Participate in care coordination meetings, contributing clinical insights on medication management and patient responses.

Medication Oversight and Safety

  • Oversee the entire medication management cycle — prescribing, dispensing, administering, monitoring, and documentation — in accordance with facility and DOH standards.
  • Manage medication ordering and coordination with external pharmacies.
  • Verify medication availability and proper storage at patient homes, ensuring safe handling and compliance with cold chain requirements where applicable.
  • Monitor and record any medication discrepancies, side effects, or adverse drug events and ensure appropriate reporting.
  • Conduct periodic audits of medication records, expiry dates, and storage compliance at patient sites.
  • Maintain clinical accuracy in documentation and reporting related to medication management.
  • Oversee the timely refilling and delivery of medications to patients’ homes.
  •  Provide education to patients and their families on medication usage and adherence.

Clinical Support and Monitoring

  • Support the bedside nurse in medication preparation and administration guidance, especially for complex or high-risk drugs (e.g., anticoagulants, erythropoiesis-stimulating agents).
  • Review patient charts and dialysis flow sheets to identify trends in medication response, adverse effects, or compliance issues.
  • Assist physicians by providing detailed nursing observations and recommendations related to pharmacologic treatment plans.

Patient and Family Education

  • Educate patients and caregivers on medication schedules, purposes, and potential side effects, reinforcing adherence to prescribed regimens.
  • Counsel patients on self-administration techniques, medication safety at home, and the importance of compliance with dialysis-related medications (e.g., phosphate binders, antihypertensives).
  • Provide written and verbal instructions to ensure continuity of medication management in the home setting.

Documentation and Quality Assurance

  • Maintain complete and accurate documentation of all medication transactions, reconciliations, and communications.
  • Support audits, incident reporting, and quality improvement initiatives related to medication management.
  • Ensure compliance with facility policies, DOH, and international medication safety standards.
  • Participate in continuous education and training sessions to stay updated on renal pharmacology and medication safety practices.
  • Contributes as part of the multidisciplinary team, in the development, implementation and maintenance of policies, procedures, standards and protocols.
  • Adheres to the requirements of the Occupational Health and Safety Management System.
  • Adheres to the requirements of the Occupational Health and Safety requirements of DOH & other regulatory authority.

Qualifications

7. QUALIFICATION, EDUCATION AND EXPERIENCE

EducationBachelor of Science in Nursing from an accredited school of nursing in home country.Active Basic Life Support Training- CertifiedCritical care qualificationBLS (for adult patients)
Experience2 to 6 years working experience as Registered Nurse 
Certification and LicensureCurrent valid registration in country of training and/or country of last employment (Local License-PRC)DOH LicensedEligible for DHA / MOH licensing
